Field Case Management
Prolonged medical treatment, complicating factors affecting the return-to-work, or catastrophic injuries are just some of the flags for field based case management. OMCA's field based case management provides four-point telephonic contacts within 24 hours of assignment - employee, provider, employer, and adjuster. Nurse case managers provide verbal reports to adjusters within 24 hours of an on-site visit and provide a written report within 48 hours of visits or monthly reports on long term assignments.

Discharge Planning
Set up home health; determine need for medical supplies and assistive devices.
Home Support Analysis
Nurse makes visit to patient's home to look at the support system, accessibility issues and to identify the need for caregiver assistance.
Patient Compliance
Monitors the patient's compliance with follow-up doctor visits, medication and restrictions.
Work Place Accommodations
Works with employer to accommodate restrictions when employee can safely return to work.
ADA Compliance
